General surgeon for Islamabad patients

Health Care Hospital sits in Satellite Town, twenty to forty minutes from most of Islamabad depending on where you start and when you set off. Single incision gallbladder surgery with no gas and same day discharge, at a fraction of what the same operation costs in the private hospitals along the Expressway.

Why cross the city for surgery

Islamabad has good hospitals and no shortage of surgeons, so a patient in F-10 or Bahria Enclave choosing to drive to Satellite Town for an operation is making a deliberate decision. Usually it comes down to one of three things.

The technique. Most units in the twin cities remove a gallbladder through four incisions with carbon dioxide pumped into the abdomen. The gas presses on the underside of the diaphragm, and because the diaphragm shares its nerve supply with the shoulder, patients wake with referred pain in the right shoulder and chest that lasts days and has nothing to do with the wound. The approach here is one incision of 1 to 2 cm, no gas at all, and no division of abdominal muscle. Patients avoid that pain entirely, and because muscle is left intact the risk of a port site hernia later is close to zero.

The cost. The same operation in a large private hospital in Islamabad can run to several times what it costs here, and the difference does not buy you a better gallbladder outcome. It buys a lobby. A number of the Islamabad patients in this practice arrived after being quoted a figure elsewhere that made them look further afield. What actually determines the cost is set out here.

Continuity. This is Dr. Khan's own hospital, not a facility he rents theatre time in. The surgeon who sees you in clinic operates on you, the anaesthetist and theatre team are the same people every week, and a patient who telephones in the evening reaches someone with the notes in front of them. In a large institution the consultant you met may not be the one holding the instruments.

Getting here

Routes in from across Islamabad

Health Care Hospital is on Holy Family Road in Block F New Katarian, Satellite Town, just off Murree Road on the Islamabad side of Rawalpindi. That position is the whole point: you reach it without going anywhere near the old city. Times below assume you are not setting out at half past eight in the morning.

Blue Area, Aabpara, G-6 and G-7

Straight down through Faizabad and onto Murree Road. The most direct approach of any in the city.

About 20 minutes

G-9, G-10, G-11 and Karachi Company

Via Kashmir Highway and the Ninth Avenue corridor to Faizabad, then Murree Road.

25 to 30 minutes

F-7, F-8, F-10 and F-11

Kashmir Highway eastbound, then down through Faizabad. Longer at peak hours, when going via the Expressway can be quicker.

30 to 40 minutes

I-8, I-9 and I-10

Among the shortest journeys in Islamabad. Down through Faizabad and onto Murree Road, or via Peshawar Road from I-10.

15 to 25 minutes

PWD, Soan Garden, Gulberg Greens and the Expressway belt

Islamabad Expressway inbound to Faizabad, then Murree Road. Straightforward outside rush hour.

25 to 35 minutes

Bhara Kahu, Bani Gala and Simly Road

Down through Quaid-e-Azam University Road and Faizabad. The one part of Islamabad where the journey in is genuinely variable.

35 to 50 minutes

Tarnol, Golra and the G-15 side

In along the GT Road and Peshawar Road rather than through the sectors, which avoids the middle of the city altogether.

30 to 40 minutes

Rawat, Koral and Chak Shahzad

Expressway inbound. Closer to Satellite Town than to most of central Islamabad.

25 to 35 minutes

The Metro bus also runs the length of Murree Road with stations at Rehmanabad and Chandni Chowk, both within easy reach of Holy Family Road. For a consultation that works well and avoids parking altogether. On the day of surgery you need a private vehicle and somebody with you, since travelling home alone after a general anaesthetic is not safe.

Practical

Same day surgery at this distance

Twenty to forty minutes is close enough that day case gallbladder surgery is entirely routine for Islamabad patients. You come in fasted in the morning, have the operation, and go home the same evening.

Two pieces of planning make the difference. Arrange a driver before the day rather than on it, because you cannot drive yourself after a general anaesthetic and you will be drowsy for several hours. And have a responsible adult stay with you the first night, not as a formality but because somebody needs to notice if you are not right and be able to telephone.

Fasting starts from home, not from the hospital door: nothing to eat for six hours before surgery, no clear fluids for two. If you are on the road early for a morning slot, plan the window around your departure.

Overnight admission is the better choice regardless of distance for older patients, anyone with significant heart or lung disease, poorly controlled diabetes, or where the gallbladder proves more difficult than expected. That call is made on the day by how you actually are rather than assumed in advance.

Follow up

Review at about a week to check the wound and go through the histopathology report on the removed gallbladder, which is sent as routine on every specimen. At this distance coming in person is straightforward, and anything that worries you sooner can be seen the same day. The sutures are absorbable, so there is no separate trip to have stitches removed.

If you work in Islamabad and need to plan around it

Desk work is usually realistic after three to five days, and driving after about a week, once you could perform an emergency stop without hesitating because of the wound. Anything involving lifting should wait two to three weeks. Tell the clinic what your work involves and the timing of surgery can be arranged around it.

Before you decide

Whether you need the operation at all

A number of Islamabad patients arrive having had stones found incidentally on a scan done for something else, a corporate medical, a check up, an unrelated abdominal complaint. They have been told they need surgery and have come for a second opinion, which is exactly the right instinct.

Stones that have never caused symptoms usually do not need removing. Community studies following people with silent gallstones show the majority never develop trouble. If you have never had the attack, observation is often the better answer and you will be told so.

What changes the picture is the pain itself. Biliary colic, a severe steady pain under the right ribs or in the upper abdomen coming on after a rich meal, spreading to the back or the right shoulder blade, lasting from half an hour to a few hours and then fading. Once those attacks begin they recur, and each one carries a chance of progressing to an infected gallbladder, a stone lodged in the bile duct, or pancreatitis. Those arrive as emergencies at times nobody chooses.

There is a third group worth naming, because it is common among office workers here. Patients whose real problem is acid reflux, gastritis or a functional bowel disorder, who also happen to have silent stones. Removing the gallbladder will not touch their symptoms, and they will be left with the same complaint and one fewer organ. Sorting out which group you fall into is what the consultation is for. Questions

From patients in Islamabad

Is it worth driving to Rawalpindi rather than using a hospital here?

That is your decision and it depends on what you are optimising for. The honest case is the technique, the cost difference for the same procedure, and the fact that the surgeon you consult is the one who operates. If your priority is a hospital five minutes from your house, that is a legitimate priority too.

How long will I be at the hospital on the day?

Plan for most of the day even though the operation itself is 30 to 60 minutes. Admission and preparation come first, then theatre, then several hours in recovery before discharge criteria are met. Arriving in the morning and leaving in the evening is the usual pattern.

Can I have the consultation and surgery on the same visit?

Not usually, and at this distance there is no need to compress it. The consultation establishes whether you need surgery and what preparation your case requires, the tests are done, and the operation is then scheduled. Being half an hour away means two short trips rather than one long one.

I had a scan at a lab in Islamabad. Will it be accepted?

Yes, bring the report and films. A repeat is only requested if it is unclear, several months old, or your symptoms have changed. You can also send a photograph of the report on WhatsApp beforehand so the clinic can tell you whether this needs an urgent slot or a routine one.

Do you deal with insurance or company panels?

Ask the clinic directly about your specific panel or insurer when you telephone, since arrangements differ. Bring your card and employer letter to the consultation if you have them.

My parents live in Islamabad and are elderly. Is surgery still reasonable?

Age by itself is not a barrier. Heart, lung and kidney function, diabetic control and general fitness determine the risk, which is what the pre-operative assessment measures. In older patients an emergency admission with an infected gallbladder is considerably more dangerous than a planned operation, so age is often an argument for treating sooner. Plan on an overnight stay rather than same day discharge.
